Patients with severe mental illness, learning disabilities, and/or autism at the Humber Centre face significant health inequalities, including higher rates of obesity and shorter life expectancy, which the Primary Care team is addressing through individualised health improvement programs.
The Humber Centre has successfully implemented a Primary Care service model to address health inequalities among its inpatient population. Notable points from the document include:
- Improved Health Outcomes: Implementation of the Primary Care service model and the “My Health” approach led to significant health improvements, including 10 out of 12 pre-diabetic patients no longer being pre-diabetic within the first year.
- Obesity and Weight Management: In response to the increased rates of diabetes and obesity post-COVID-19, a formal Forensic Services Obesity and Weight Management Care Pathway was launched, led by Primary Care.
- Innovation: The service is one of only two in the country with a dedicated Primary Care service for forensic inpatients and has been noted for its innovative approach and the autonomy given to the team to explore new models.
