Neighbourhood Health Partnerships are a model of delivering coordinated health and care that focuses on integrated, community-based support tailored to local needs. Neighbourhood Health Partnerships are made up of a multidisciplinary team of professionals and organisations including primary and secondary care providers, local authorities and VCSE organisations.
Neighbourhood Health Partnerships aim to support healthier communities through:
• Improved health and wellbeing
• Reduced inequalities
• Improving people’s experiences of health and care

Key Activities:
- Developing the maturity of the seven Neighbourhood Health Partnerships
- Resetting the strategic focus from Integrated Neighbourhood Teams to Neighbourhood Health Partnerships
- Transitioning the Neighbourhood Health Partnership Steering Group from being ICB led to Provider ownership
- Within each neighbourhood: convening stakeholders to grow the partnerships, developing strategic plans, aligning with existing strategy and infrastructure, commencing or continuing the implementation of projects and priorities
