Managing complexity and crisis is one of the key challenges faced by the care sector in the region. Through extensive analysis it has been identified that there is a gap in complex care provision and care providers are increasingly faced with managing complexity and crisis in their residents.
During and post pandemic requests for one-to-one care and support significantly increased and have since remained consistently high, one-to-one is being used to support people with the most complex care and support needs. Care of this type can be restrictive for the individual and additionally is of exceptional high cost.
Through the Complex Care Programme, work is being developed to ensure that it is person-centred, where staff actively listen and seek to understand individual’s, including how they communicate their needs, emotions or distress, which can help reduce restrictive practices. The most effective support is delivered through care plans that are tailored to each person’s specific needs, informed by their experiences of trauma, and regularly reviewed to ensure they remain relevant and responsive. Our ambition is to deliver outcomes for individuals with the most complex care needs through a collaborative approach. By providing high-quality care across Adult Social Care and Health, we aim to ensure that support is both person-centred and cost effective, delivering best value for individuals and services alike.
This programme has three workstreams that are shaping the future of care to ensure that it fulfils this ambition:
- One-to-one care and support assurance
- Older people care home crisis in-reach
- Section 117 development (now moved to business-as-usual delivery)
Key Activities:
- Assurance reviews of one-to-one care packages have resulted in a reduction in the number of clients in receipt of one-to-one care and a reduction in the number of one-to-one hours delivered
- Development of a complex care/ one-to-one protocol for supporting the implementation and review of care
- Crisis in-reach pilot project is supporting older people’s residential homes to manage complexity and crisis
- S117 ordinary residence review and register development
- S117 practice guidance produced
