Purpose
The East Riding Population Health Steering Group works to embed population health approaches across the health and social care system. It focuses on three key areas:
1. Intelligence
Develop an approach based on the ‘intelligence triangle’:
- Quantitative data
- Insights from local practitioners
- Voices of local people
These resources feed into the Joint Strategic Needs Assessment (JSNA) for analysis and practical use.
2. Culture
Drive a cultural shift so population health becomes part of everyday practice. This involves:
- Sharing best practice
- Building knowledge
- Applying ideas in real-world settings
3. Delivery
Move population health approaches towards ‘business as usual’. Practical application will make the change happen.
Intelligence Activities
- Integrate intelligence from JSNA, Pharmaceutical Needs Assessment, Adults Local Account, Market Position Statement, Children’s Sufficiency Statement, Joint Strategic Intelligence Assessment, and community asset reviews.
- Analyse interdependencies between statutory partners to support integrated commissioning.
- Provide intelligence support to Neighbourhood Health Partnerships (NHP).
- Map service flows and demand. Identify system links and gather engagement insights to inform redesign.
- Multimorbidity Intelligence: Use population data to strengthen community resilience, promote independence, and reduce reliance on statutory services.
- Lead joint intelligence models, assess health and care needs, and shape services using evidence-based approaches.
Culture Activities
- Expand the East Riding Population Health Community of Practice to share learning and best practice.
- Develop and deliver a comprehensive Population Health Training Programme.
- Promote ongoing training and development through the Community of Practice to support continuous improvement.
